The AI Didn't Go Rogue. You Rubber-Stamped It.
The scariest thing your AI agent will ever do is exactly what you approved. Not because it misunderstood you, but because you stopped reading the part where you said yes.
Here is the failure that never makes the incident report: an agent proposes an action, a human clicks approve, it runs, it goes sideways, and the room asks how the model made such a bad call. Wrong question. A person approved it. The person just did not read it. That is approval fatigue, and it is the quietest dangerous thing in your whole operation.
An approval you did not read is worse than no approval at all. No approval is honest. A rubber-stamped approval is a forgery: it writes a human name onto a decision no human actually made, and the audit trail points at a person who was really just tapping a button between meetings.
The fix is not a smarter model. A smarter model handed to a rubber-stamping operator just gets its worse ideas approved faster. The fix is fewer gates, each one worth a full minute of real attention, and a brain that remembers which decisions actually mattered so it stops asking about the forty that never did.
The rogue agent is a movie plot. The rubber stamp is your Tuesday. Fix the Tuesday.
